Shawn Smith LMSW

Certified Care Planning Specialist

Shawn, a Michigan transplant, obtained his Bachelor of Science degree in Psychology in 1998 and his Master of Social Work degree in 2004 from Arizona State University. Since then, he has committed his life to assisting Arizona’s most vulnerable and underserved populations, which include individuals within the Hospice, Palliative, homeless, substance-addicted, elderly, and severely mentally ill communities. Over the course of the past 15 years, Shawn has primarily directed his focus and efforts toward Arizona’s elderly population.

For a decade, Shawn served as a Hospice Social Worker, aiding those with terminal illnesses by providing them comfort and ensuring their dignity during their final moments. With the intention of making a more substantial impact on the lives of Arizona’s seniors, Shawn extended his efforts to a broader scale. In his capacity as the Director of Community Outreach, he was deeply involved in initiatives designed to support individuals affected by abuse, neglect, and financial exploitation. To this end, he established and led numerous support groups, specifically dedicated to honoring Veterans and those grappling with terminal illnesses. Additionally, Shawn hosted various community and professional education panels, contributing his expertise and insights to disseminate knowledge among these audiences.

Beyond his professional roles, Shawn continues to be an active volunteer, offering support, carrying out minor home repairs, imparting education, and furnishing resources to the local senior community. His enduring dedication exemplifies his commitment to enhancing the well-being and quality of life for Arizona’s seniors, leaving an indelible mark on the communities he serves.
