Michelle Adamczyk

Certified Care Planning Specialist

With over 20 years of experience in the world of senior care, Michelle is a compassionate, seasoned professional who is keenly focused on providing appropriate care solutions for seniors and their loved ones. 

Michelle excels in discovering what is important and necessary for the families she is honored to help.  She prides herself on providing comprehensive options that meet the individual needs of her clients. 

Originally from northeast Ohio, Michelle worked as a hospital clinical liaison for Saber HealthCare Group, a prominent conglomerate of skilled nursing facilities, where she facilitated hundreds of discharges from the Cleveland Clinic, University Hospitals and MetroHealth. 

She was then recruited by University Hospitals, one of the nation’s leading healthcare systems. Working with the case management team in the role of Patient Care Navigator, Michelle assisted patients with safe, smooth discharge transitions to skilled nursing, assisted living, acute rehab and home with home health services.  

Upon moving to AZ, she worked for a nationwide provider of Assisted Living / Independent Living communities in Scottsdale.  

Michelle understands the complexity of navigating the world of senior living and care.  She is grateful to be a resource for families, providing care, comfort and empathy.  She will guide you and hold your hand through the process, and as she says “help you put the pieces of the puzzle together.” 

Michelle enjoys hiking, spending time with her family and her sweet beagle, Buddy.  You can find her on football Sundays cheering on the Cleveland Browns!
